I understand that misrepresentations or falsifications herein or in other documents completed or submitted by the applicant will result in dismissal, regardless of the date of discovery by the camp.

I understand that, if hired:

- I will be hired as an independent contractor

- I will be responsible for my own medical insurance, and provide proof of insurance. (If I do not have medical coverage, I will be responsible for my own medical costs if admitted into a hospital or for walk-in clinics appointments)

- I will be required to submit a recent (within 1year) medical record, signed by a Physician

- I will be required to provide a background check from my local police station

- I understand a Criminal and Sex Offender Record would be pulled up on me and will provide the necessary information to pull up such record

- I will provide a copy of my drivers license and/or other form of government ID.

- I acknowledge that I am legally allowed to work in the US.

- I understand that I will be working at a NIKE affiliated camp and will not wear or promote a competition's brand, and will do my best to wear Nike brand or non branded clothings.
